Job Summary

The Licensed Nurse is responsible for providing direct patient care under the supervision of a registered nurse and in concert with our values: trustworthy, capable, compassionate, proactive, and called. Essential Functions

Patient Care

  • Provide direct patient care in accordance with the state Nurse Practice Act.
  • Implements plan of care initiated by the registered nurse.
  • Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Use and prepare equipment and supplies safely and effectively, consistently adhering to aseptic technique.

Communication & Coordination

  • Prepares clinical notes in accordance with professional licensing body and internal policy using the company's electronic medical record and submits documentation timely.
  • Attends and contributes to in-services, case conferences, and other meetings as required by agency policy to ensure coordinated and comprehensive plans of care for the patients of the agency.
  • Interacts with team members in a positive and professional manner.
  • Communicates plan of care to referring physician and other agency professionals.
  • Maintains contact/communication with other personnel involved in the patient's care to promote coordinated, efficient care

Additional Responsibilities

  • Participates in personal and professional growth and development.
  • Performs other duties as assigned by the registered nurse.

Qualifications

  • Graduate of an accredited Licensed Practical Nurse or Licensed Vocational Nurse program.
  • Three (3) years nursing experience, preferred. Community, home health or medical/surgical experience is preferred.
  • Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
  • Possess and maintain valid CPR certification.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication and organizational skills.
  • Licensed driver with reliable transportation.
