Licensed Practical Nurse - Home Health PRN

Essential Functions
- Provide direct patient care in accordance with the state Nurse Practice Act.
- Implement plan of care initiated by the registered nurse.
- Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
- Use and prepare equipment and supplies safely and effectively, consistently adhering to aseptic technique.
Communication & Coordination
- Prepare clinical notes in accordance with professional licensing body and internal policy using the company’s electronic medical record and submit documentation timely.
- Attend and contribute to in-services, case conferences, and other meetings as required by agency policy to ensure coordinated and comprehensive plans of care for the patients of the agency.
- Interact with team members in a positive and professional manner.
- Communicate plan of care to referring physician and other agency professionals.
- Maintain contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care.
Additional Responsibilities
- Participate in personal and professional growth and development.
- Perform other duties as assigned by the registered nurse.
Qualifications
- Graduate of an accredited Licensed Practical Nurse or Licensed Vocational Nurse program.
- Three (3) years nursing experience, preferred. Community, home health or medical/surgical experience is preferred.
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
- Possess and maintain valid CPR certification.
- Comply with accepted professional standards and practice.
- Demonstrate good verbal and written communication and organizational skills.
- Licensed driver with reliable transportation.
