About the Role:
Join our team of dedicated professionals at Better at Home, committed to providing exceptional care to our patients in the comfort of their homes. We are seeking compassionate full-time and part-time Licensed Practical Nurses (LPNs) to join our Henryetta care team. Working collaboratively with our supervising Registered Nurses, your expertise will make a significant impact on the lives of those we serve by helping local patients safely manage chronic conditions and achieve their optimal health at home.
Details:
- Full-Time: Approximately 25-30 visits per week
- Part-Time: Variable schedule based on patient needs and availability
- Compensation: $43.00-$45.00 per visit
- Provide skilled nursing care in the home as ordered on the plan of treatment and nursing care plan.
- Evaluate the significance of assessment findings and communicate pertinent information about the patient's clinical status and ongoing needs to the care team in a timely manner.
- Adequately assess and reassess pain, utilize appropriate pain management techniques, and educate patients and families regarding pain management.
- Maintain accurate medical documentation and update medical records in a timely manner.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Educate patients and their families about health management and disease prevention.
- Current LPN license in the state of Oklahoma in good standing.
- Graduate of an accredited practical nursing program with current CPR/BLS certification.
- Knowledge of pain management techniques and patient care best practices.
- Familiarity with medical documentation practices and electronic health record (EMR) systems is preferred.
- Strong interpersonal skills with the ability to build rapport with patients and families.
- Valid driver's license and reliable transportation for local home visits.
