Matrix Medical Network seeks a licensed Nurse Practitioner to conduct in-home comprehensive health assessments, reviewing medical history, medications, and risk factors. The provider will administer tests, coordinate with PCPs and care teams, and deliver services at the member’s residence, including SNFs.
Qualifications include NP licensure, board certification in relevant specialties, BLS/ACLS, and 1 year of NP experience preferred.
