Role DescriptionThis is a full-time, on-site Registered Nurse role based in Eufaula, OK. The Registered Nurse will provide direct skilled nursing care to clients in their homes, including assessments, wound care, medication administration, and monitoring of vital signs and overall health status. The role includes developing and updating individualized care plans, collaborating with physicians and other healthcare professionals, and educating clients and families on disease management, medications, and home care procedures.
The nurse will document all care accurately, ensure compliance with regulatory standards, and support a safe home environment. The position also involves participating in case conferences, responding to changes in client condition, and promoting high-quality, patient-centered care.
- Active Registered Nurse (RN) license in the state of Oklahoma and current CPR/BLS certification.
- Experience in home health, hospice, or community nursing, or strong interest in home-based care.
- Proficiency in clinical assessment, care planning, medication administration, and wound care.
- Strong documentation skills and ability to use electronic health records or comparable systems.
- Excellent communication and interpersonal skills to collaborate with clients, families, and care teams.
- Ability to work independently, manage time effectively, and prioritize multiple client needs.
- Valid driver’s license, reliable transportation, and ability to travel locally to client homes.
- Associate or Bachelor’s degree in Nursing; prior experience in home health or geriatrics is a plus.
