- JOB SUMMARY:
Under close supervision, responsible for assisting with individual and/or group services, which include assessment, coordinating, planning, advocacy, linkage, referral, education, crisis intervention, resource acquisition, and monitoring in a culturally sensitive way. Provide point of contact and work in partnership with others as part of a multidisciplinary team to provide a set of social service functions that helps clients access the resources they need to recovery.
Services are provided in the office or in the community-based on selected program. Resume Required. Duties and responsibilities may include:
- Assist with case management and rehabilitation services using a strengths-based, welcoming, recovery-oriented, client-driven approach for an assigned group of clients.
- Observe the development of strengths-based comprehensive intake screening, assessment; treatment plans; modification of treatment plans.
- Develop a resource guide and learn to collaborate with community resources to assist with clients' needs.
- Participation in multidisciplinary team meetings.
EDUCATION & QUALIFICATIONS - CORE:
Enrolled in Bachelor's Degree in the social sciences field is required. Must have a strong commitment to adhere to clients' right to have access to helpful, adequate, competent, and continuous supports and services. Must possess a valid Oklahoma Driver License and satisfactory driving record and use personal automobile to travel to locations other than primary office.
OTHER INFORMATION SAFETY SENSITIVE JOB CLASSIFICATION:
This job is classified as a "safety-sensitive" position as defined by the Oklahoma Medical Marijuana and Patient Protection Act. Due to the "safety-sensitive" classification, an employee in this position would be subject to drug and alcohol testing, including random testing.
