Job Coach, Social Work, Career & Life Skills Coaching

Home of Hope, a faith-based residential care facility in Georgia, seeks a Job Coach to support homeless children and their young mothers. The Job Coach provides one-on-one and group coaching to build life, job readiness, and educational skills that lead to stability and independence. Responsibilities include developing individualized action plans, facilitating life-skills and employment workshops, coordinating with case managers and therapists, and connecting families to community resources.

The ideal candidate brings nonprofit or social services experience, strong documentation and communication skills, and a heart for serving at-risk youth in a faith-centered, team-oriented environment.

Responsibilities

• Provide one-on-one coaching to young mothers and youth to build life, employment, and educational skills
• Develop individualized action plans focused on stability, independence, and goal achievement
• Facilitate group workshops on life skills, job readiness, and personal development
• Coordinate with case managers, therapists, and educators to support holistic care plans
• Track client progress, maintain accurate documentation, and prepare reports
• Connect clients with community resources, training, and employment opportunities
• Model positive, faith-aligned behaviors and promote a safe, nurturing environment
• Support program intake, assessments, and discharge planning
• Communicate regularly with internal staff and external partners about client needs
• Participate in staff meetings, trainings, and continuous improvement initiatives
