Meadowbrook Rehabilitation Hospital, located in Gardner, KS is a specialty rehabilitation hospital that provides acute inpatient rehab and brain injury rehabilitation. We are looking for a case manager to join our team.

SUMMARY

The case manager is the resource person in the area of inpatient case management and discharge planning.

DUTIES PERFORMED
  • Conducts psychosocial history to determine pre‑injury issues that need to be addressed.
  • Establishes and carries out an individualized treatment plan upon completion of psychosocial history, if applicable.
  • Provides counseling to clients, families, and/or significant others.
  • Develops and implements family and children support groups as needed.
  • Schedules, arranges, and moderates the program for family education groups.
  • Participates in team rounds and family conferences as scheduled through the case manager.
  • Maintains records and prepares reports according to facility procedures.
  • Participates in and documents internal and external professional educational activities to enhance personal and professional growth.
  • Performs public relations tasks as directed by supervisor.
  • Coordinates the client's schedule with physical therapy, occupational therapy, therapeutic recreation, nursing service, speech and language pathology, behavioral specialists, and other disciplines as needed.
  • Is an integral part of the interdisciplinary team.
  • Key component in assisting in patient discharge planning.
  • Conducts post discharge follow‑up as needed.
  • Any other tasks as assigned by the immediate supervisor.
SPECIFICATIONS
  • Bachelor's in Social Work or related field Preferred
  • Formal Education/Formal Training:
    Degree/license in healthcare Preferred
  • Job-Related

    Experience:

    Minimum of two years’ experience in a hospital/health care setting, preferably in an Inpatient Rehab Facility (IRF)
