Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. PACEProgram
The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services.
By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.
A Physical Therapist Assistant (PTA) at Midland Care’s PACE program works under the direction of a licensed physical therapist to help older adults maintain or improve their strength, balance, mobility, and independence. The PTA provides therapeutic exercises and functional activities such as walking and transfers, helps participants use mobility equipment safely, and provides education on exercises and fall prevention. PTAs may work with participants in the PACE center, their homes, or other community settings.
They also monitor participants’ progress, document treatment, and communicate changes or concerns with the physical therapist and interdisciplinary care team. Overall, the PTA’s role is to help PACE participants remain safe, active, and as independent as possible in their daily lives.
- Graduation from an accredited Physical Therapist Assistant program with an associate degree
- Current Kansas PTA licensure
- Knowledge of physical therapy and rehabilitation principles
- Ability to provide treatment under the direction and supervision of a licensed physical therapist
- Ability to communicate effectively with participants, caregivers, and other members of the care team
- Ability to complete accurate and timely documentation
Ability to work effectively as part of an interdisciplinary care team - CPR/BLS certification
- Physical ability to assist participants with mobility, including standing, walking, bending, and transfers
- Previous experience working with older adults or geriatric populations
- Experience in PACE, home health, long-term care, or skilled nursing
- Experience working with chronic conditions and age-related health concerns
- Knowledge of fall prevention and safe mobility techniques
- Experience providing education and training to participants and caregivers
- Familiarity with adaptive equipment and assistive devices
- Strong organization and time-management skills
- Strong communication, interpersonal, and documentation skills
