Description
  • Communicate with providers and assist patients with continuity of care by connecting to
    community resources and other necessary services.
  • Maintain routine care management records with accurate documentation.
  • Referrals for services, screening and scheduling, creating chart and verification of demographics.
  • Verify patient financial responsibility, documentation and notification to patient.
  • Comply with standards of patient confidentiality.
  • Monitor progress toward treatment plan outcomes to identify and overcome barriers.
  • Provide technical assistance, consultation, and culturally appropriate health information to improve
    delivery of services to underserved patients and their families.
  • Identify patients eligible for various assistance programs(including Medicaid and Kan Care) and assist
    patients in accessing these programs.
  • Identify available resources in the community and create, collect, maintain and update all resource
    materials for patient and provider access.
  • Maintain positive working relationships with referring agencies, community resource
    organizations and other providers.
  • Participate in community partner meetings, trainings and staff meetings.
  • Coordinate with the marketing department to participate in outreach and community
    events/activities.
  • Represent HPC in the community, including community meetings, presentations and events to build and
    strengthen community partnerships.
  • Assist the Director of Behavioral Health to assess current patient care procedures identifying areas of
    needed and improvement strategies.
  • Assist the Director of Behavioral Health in quality improvement, development and evaluation of the
    behavioral health department, including tracking data, scheduling patients for specialty services and any
    other needs related to monitoring and execution of Behavioral Health programs and services.
  • Assist with translation services. (if applicable).
  • Other duties as assigned.

  • Requirements Qualifications
  • Ability to relate well with patients and other health care personnel with cultural sensitivity.
  • Willingness to educate patients and the community regarding HPC services, health
    maintenance and the role of the health care team.
  • Ability to work in a fast paced, challenging environment under pressure.
  • Commitment to expanding access to care for the underserved.
  • Education and Experience
  • High School Diploma, required
  • Bilingual Spanish, required
  • Bachelor’s degree in health care, psychology, social work, or related field, preferred.
  • Experience working in healthcare or community health setting, preferred
  • Typical Physical Demands
    • Prolonged sitting or standing may be required.
    • Ability to lift 25 pounds; occasionally up to 60 pounds.
    • Those physical movements and the degree of mobility, manual dexterity and hand-eye coordination
      normally associated with normal medical office will be performed on a repetitive basis.
    • The ability to distinguish letters and symbols as well as the ability to utilize telephones,
      computer terminals and copiers is required.
    • Work under stressful conditions as well as irregular hours may be required.
    • Frequent exposure to communicable disease, body fluids, toxic substances, medicinal preparations,
      radiation, and other conditions common to a clinical environment may routinely be encountered.

    Under the direct supervision of the Director of Behavioral Health, the Behavioral Health Coordinator is
    responsible for a variety of administrative and care management activities with an emphasis on ensuring patients
    receive and access resources monitored by continuous communication.
