Position Title

RN - Integrated Nurse Case Manager - PRN Olathe Hospital

Position Summary / Career Interest

The Integrated Nursing Case Manager, under the direction of the Director/Manager of Case Management, provides care/service safely and efficiently for a full range of services to patients of all ages and their families. The primary role is to collaborate, communicate and facilitate coordination of services during hospitalization and post‑hospitalization as established by the health‑care team. The coordinator concurrently facilitates proactive patient care services to ensure that care is appropriate, timely, and cost‑effective and achieves the desired outcome.

The Integrated Nursing Case Manager coordinates the care and service of selected patient populations across the continuum of illness, promotes effective utilization and monitoring of health‑care resources, and guides all disciplines toward positive quality outcomes.

Responsibilities and Essential Job Functions

Accept responsibility for achieving optimal outcomes within scope of practice; follow policies, procedures and standards; comply with Corporate Compliance program; assume responsibility for risk and safety issues; take call as required; perform job responsibilities with accountability; maintain licensure; advocate for patients and caregivers; ensure patient choice and patient‑centered environment; identify and manage patient needs with referrals; ensure prudent utilization of resources;

balance cost and quality; participate in performance improvement activities; review assigned Outpatient Observation patients; conduct admissions and stay reviews; determine medical necessity; co‑ordinate with physicians, insurance companies and external payers; facilitate discharge planning and resource referrals; monitor financial responsibilities; maintain accurate communication; facilitate interdisciplinary care coordination; manage transitions of care; document in Case Management system; prepare for and participate in daily Interdisciplinary Care Coordination huddles;

collaborate with families and community partners; be accountable with measurable cost‑effective outcomes; present at Weekly Length of Stay meetings; prioritize activities by payer reimbursement; intervene per policy on over‑utilization; assist in appeal processes; provide crisis intervention; be on call 24 hours a day, seven days a week; continually improve processes; monitor outcomes; identify improvement opportunities; assist medical staff with peer reviews;

serve as resource for performance improvement; complete system requirements; participate in professional development; pre‑cise new team members; demonstrate flexibility and teamwork; provide coverage to other services; and identify and report opportunities for quality improvement.

Required

Education and Experience
  • High School Graduate
  • Associate Degree Nursing (Preferred)
  • Bachelor Degree Nursing
  • 2 or more years clinical experience
  • 2 or more years clinical experience in a health care setting
Required Licensure and Certification

Licensed Registered Nurse (LRN) – Single State – State Board of Nursing;
Registered Nurse in Kansas. Must hold a current active multistate state RN license in good standing in the state in which the candidate lives, or an active single state RN license in good standing in the state of employment. Staff hired prior to 2/1/2024 are required to obtain a multistate state RN license from the state in which they live at the time of their next RN license renewal by 3/31/2026.

Basic

Cardiac Life Support (BLS or BCLS)

American Heart Association (AHA)

Knowledge Requirements
  • Ability to proficiently read and document in electronic medical record
  • Basic typing and word processing ability
  • High degree of cognitive skill – strong attention to detail
