The Nursing Assistant (NA) serves as caregiver to the resident during the scheduled work period in long‑term care. Under the supervision of a registered nurse (RN), the NA provides resident‑centered nursing care and daily‑living assistance, including the following responsibilities:

• Provides resident‑centered nursing care and daily living assistance to assigned resident under RN supervision.
• Delivers age‑appropriate care based on the resident’s physical and psychological needs as per the care plan.
• Assists with basic health‑care needs, including bathing, toileting, grooming, dressing, vital sign recording, and psychosocial support.
• Assists residents in transferring, repositioning, and walking using correct transfer techniques and equipment, and performs range‑of‑motion and passive exercises.
• Completes food preparation and dining tasks, creates a positive dining experience, and assists residents with meals and snacks.
• Assists nursing staff with procedures as needed.
• Documents resident interactions and outcomes related to resident care, behavior, activity, and dietary; communicates changes to the charge nurse.
• Collects non‑invasive body‑fluid specimens and gathers vital signs.
• Provides housekeeping and laundry services to maintain clean living areas.
• Assists with wellness and recreational programs and encourages resident participation.
• May participate in departmental or organizational committees and task forces as required.

• High school graduate or GED preferred; not required.
• Minimum age 16.
• Prior clinical or nursing assistant experience in long‑term care preferred.
• Certified Nursing Assistant (CNA) certification required, to be obtained within four months of hire or through an enrollment/endorsement program; training and certification provided by the facility.
• Successfully completes yearly department‑specific competency testing and maintains at least 12 hours of continuing education credits annually.
• Maintains all required certifications mandated by the department.
• When applicable, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
• In facilities where driving is part of the role, classified as a Sanford Category II Driver, must submit motor vehicle report and proof of a valid driver’s license and must be medically qualified to perform the essential driving functions per Sanford Driving Policy.
