Overview

The Nursing Assistant (NA) serves as a caregiver to the resident during the scheduled work period in long‑term care, providing resident‑centered nursing care and daily living assistance under the supervision of a registered nurse (RN). The role involves delivering age‑appropriate care related to the physical and psychological needs of the resident as outlined in the care plan.

• Assist with basic health‑care needs including bathing, toileting, grooming, dressing/undressing, and obtaining and recording vital signs.
• Provide psychosocial support and other personal care to the assigned resident.
• Assist the resident in transferring, repositioning, and walking using correct transfer techniques and equipment; provide range‑of‑motion and passive exercises.
• Complete tasks related to food preparation and dining, creating a positive dining experience.
• Assist residents with meals and snacks, and provide water and supplements as appropriate.
• Support providers and nursing staff with procedures as needed.
• Document resident interactions and outcomes related to resident care, behavior, activity, and dietary.
• Communicate changes in the resident’s condition and related concerns to the charge nurse.
• Collect non‑invasive body‑fluid specimens and gather vital signs.
• Provide housekeeping and laundry services to assist residents in keeping living areas clean and operational.
• Assist in wellness and recreational programs and encourage resident participation.
• Participate in departmental or organizational committees and task forces as needed.

• High‑school graduate or GED (preferred).
• Minimum age of 16.
• Prior clinical or nursing assistant experience in long‑term care (preferred).
• Certified Nursing Assistant (CNA) certification required. Must obtain a state‑certified CNA within four months of starting the position, either by enrollment in a training program or by completing an endorsement program.
• CNA training and certification will be provided by the facility.
• Satisfactorily complete yearly department‑specific competency testing and maintain a record of continuing education credits.
• Complete a minimum of 12 hours of continuing education annually.
• Maintain all department‑specific required certifications.
• When applicable, be certified and registered with the Unlicensed Assistive Person (UAP) Registry.
• If required by the facility’s responsibilities, the role may be classified as a Sanford Category II Driver, requiring a motor‑vehicle report, proof of a valid driver’s license, and medical qualification to perform essential driving functions.
