• Coordinate and manage hospice care plans for assigned patients
• Assess patient needs and develop individualized care plans with interdisciplinary team
• Provide compassionate direct patient and family support, education, and advocacy
• Collaborate with physicians, social workers, and spiritual care providers
• Ensure compliance with regulations, documentation, and quality standards

• Active RN license in relevant state
• Hospice or palliative care experience preferred
• Strong communication, assessment, and care coordination skills
• Ability to work collaboratively within an interprofessional team
