Nurse Practitioner (NP) - Home Health Care (Northern New Jersey)

Salary: $130,000 - $140,000 per year

We are seeking a reliable, compassionate, and self-sufficient Nurse Practitioner to support home-bound patients throughout Northern New Jersey. This role is hands-on, relationship-focused, and allows you to provide consistent, high-quality care directly where patients need it most.

What You'll Be Doing
You'll be the primary provider for 5-6 home-bound patients per day, delivering high-quality care in the comfort of their homes:

• Perform assessments, vitals, and physical exams

• Diagnose health conditions and illnesses

• Prescribe and manage medications

• Maintain accurate clinical notes and visit schedules

• Build trust and provide guidance to patients and families

• Educate on conditions, prevention, and overall health habits

• Promote patient-centered, compassionate care

What You Need

• Active New Jersey NP license

• DEA license

• Malpractice coverage

• Strong communication skills and professionalism

• Ability to work independently

• Proven NP experience in primary care, ER, clinic, or nursing home settings

Salary

• $130,000 - $140,000 per year
