LPN​/LVN

As an LPN/LVN at Encompass Health, you will deliver high-quality, patient-centered nursing care in our inpatient rehabilitation hospital. Working within an interdisciplinary team, you'll provide direct bedside care, administer medications and treatments, monitor and document patient progress, and support mobility and activities of daily living. You'll educate patients and families, reinforce rehab goals, and help ensure safe, timely discharges, all within a nurturing, dynamic environment that offers strong professional growth.

Responsibilities

• Provide direct bedside LPN/LVN care to rehab patients under RN/physician direction
• Administer medications and treatments accurately and safely
• Monitor, document, and report patient status and vital signs in the EMRSupport mobility, ADLs, and rehab goals to promote safe recovery
• Collaborate with the interdisciplinary team on care plans and discharge needs
• Educate patients and families on treatment, medications, and self-care
• Maintain infection control, safety, and quality care standards
