Your Role as the Licensed Practical Nurse (LPN)As an LPN in our Assisted Living community, you’ll provide compassionate, high‑quality clinical care and support a strong caregiving team.
SummaryThe LPN is responsible for providing direct nursing care, supporting resident wellness, and collaborating with caregivers and leadership to ensure safe, compliant, and resident‑centered care within an Assisted Living environment.
Role May Also Be Known AsLicensed Practical Nurse (LPN), Assisted Living Nurse, Wellness Nurse (LPN), Memory Care Nurse (LPN), or Resident Care Nurse.
What You’ll Do- Provide direct nursing care to residents in accordance with state regulations and facility policies
- Support and guide caregiving staff to ensure consistent, high‑quality care
- Assist with resident assessments, care planning, and ongoing monitoring of changes in condition
- Administer medications and treatments per physician orders and established protocols
- Maintain professional relationships with physicians, office staff, pharmacies, and healthcare providers
- Collaborate with the Regional Nurse Consultant and community leadership to support clinical best practices
- Support orientation and training of care team members as needed
- Maintain accurate and timely clinical documentation
- Promote a safe, respectful, and home‑like environment for residents

Education:
- Graduate of an approved school of nursing
Licensure / Certification:
- Current Licensed Practical Nurse (LPN) license required
Experience &
Skills:
- Experience in assisted living, memory care, or long‑term care preferred
- Strong clinical judgment and attention to detail
- Ability to work collaboratively with residents, families, caregivers, and healthcare partners
- Effective communication and teamwork skills

- Lift up to 25 lbs. and assist with resident transfers as needed
- Push, pull, and maneuver up to 100 lbs., including resident mobility assistance
- Maneuver resident wheelchairs weighing up to 350 lbs.
