Job SummaryUnder assigned supervisor, the BH Community Outreach Coordinator is responsible for the planning, direction, and coordination of the community outreach for BH programs, including Care Coordination, Intensive Placement Stabilization, Mobile Crisis Response, and Community Crisis Response Program.
Essential Duties and ResponsibilitiesPlanning and leading community outreach for the following programs:
Care Coordination (CCS), Intensive Placement Stabilization (IPS), Mobile Crisis Response (MCR), and Community Crisis Response Program (CCRP).
- Identify existing local community networks and resources and maintain a list of those resources, including information regarding service type, population served, eligibility requirements, and other pertinent information for referral.
- Collaborate with existing local community networks throughout program coverage areas.
- Provide resource connection information for the local community networks and referral resources to the CCS staff, IPS staff, MCR staff, and CCRP staff, ensuring all staff know how to access the resources/services for their clients.
- Connect with all networks provided by HFS.
- Facilitate quarterly meetings with all identified service providers and networks willing to participate to discuss services, supports and options for areas of service gaps.
- Specific to CCS, submit a bi‑annual report to HFS detailing the strengths of the area networks and service gaps/areas of improvement.
- Specific to MCR, build and maintain connections with area emergency room directors, leaders, and staff to communicate crisis needs, concerns, and enhancements.
- Monthly organized communication will be expected.

Education
- Bachelor's degree in the field preferred
Experience
- 1‑3 years' experience in the field preferred BH Community Outreach Coordinator
Other
- Reliable transportation, valid driver’s license, and acceptable driving record with OHU required levels of coverage for auto insurance. DCFS background clearance.
- Ability to assess, interpret, and deliver care‑related information concerning the life‑span development of children and families
- Excellent written and oral communication, with ability to document in a clear, concise manner
- Highly organized, yet flexible and can handle multiple projects and or priorities well with the ability to manage tight deadlines
- Positive attitude concerning all aspects of working in a challenging environment (including significant patience and respect for children and families who can become quite angry and demanding), sensitivity to cultural needs, and ability to serve as a positive member of a working team
- Resourcefulness in researching issues, analyzing information skillfully, solving problems, and developing solutions with minimal supervision in a timely manner
