Job SummaryUnder the care coordination support worker role, you will provide community-based services to identified youth within the Systems of Care. You will collaborate with families, service providers, and the agency to develop and implement individualized, trauma-informed Wraparound plans.
Essential Duties- Target youth identified in the Pathways program and serve them in accordance with program policies.
- Apply an evidence-informed, structured approach to care coordination and adhere to agency procedures and fidelity measures.
- Integrate all services for youth and families, serving as a central communication point for service components.
- Collaborate with youth and families to develop a Plan of Care and a Crisis Prevention Plan.
- Serve a caseload referred to Pathways to Success Tier Level Wraparound and manage caseloads within the high fidelity wraparound framework.
- Complete strength-based assessments, update the IM CANS, and monitor progress toward goal achievement.
- Facilitate Child and Family Team Meetings (CFTM) at regular intervals and coordinate collaboration with service providers.
- Advocate for the youth and family in the least restrictive setting possible and implement services to avoid unnecessary inpatient or residential treatment.
- Bachelor’s degree in psychology, social work, counseling, education, or a related field from an accredited university.
- Two years of experience coordinating care, including facilitating child and family team meetings and working directly with youth and families.
- Valid driver’s license and acceptable driving record; ability to work varying hours, including evenings and weekends.
- Strong collaborative, relationship-building, and communication skills.
- Time management and the ability to consistently meet deadlines.
- Knowledge of the Illinois child welfare systems and experience working with diverse populations.
- Proficient computer skills, including Microsoft Windows.
- Effective written and verbal communication skills in English.
