Must-Have:
Inpatient acute care hospital experience is required.
Behavioral hospital experience is not considered equivalent.
Candidates with less than 2 years of experience may be screened if they possess acute care hospital experience.
The role requires floating between like units to meet facility needs.

Description:

The Care Manager RN will provide care management services.
Candidates should have experience working in an acute care hospital environment.
Behavioral health experience will not be accepted.
This role will serve a rural community with limited resources.
Primary responsibilities include care coordination, discharge planning, and needs assessment.
Candidates should be comfortable with charting in an electronic health record system.
The average daily census for the facility is 300 patients.
Weekend coverage is required every other week, with flexibility to meet facility needs.
All requests for time off must be listed at the time of submission.
Candidates must have reliable transportation.
Successful candidates should be open to the possibility of permanent employment after the designated time period.
Housing arrangements should be secured prior to accepting the position.
Compliance and completion of sensitive onboarding items, including background checks, will be essential before the start date.
