Role:
Occupational Therapist providing personalized in-home and senior living care to older adults, utilizing the Geriatric House Calls™ model.
Responsibilities- Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility, independence, and wellness.
- Provide one-on-one therapy sessions in private residences, senior living communities, and via telehealth when appropriate.
- Document patient progress accurately within the EMR system and collaborate with interdisciplinary team members.
- Educate patients, caregivers, and staff to promote safety and aging in place.
- Valid Occupational Therapy license or eligibility in the state of practice.
- Degree from an accredited OT program and proficiency with EMR systems.
- Current CPR certification and ability to meet physical demands of in-home care, including lifting, repositioning, and driving.
Autonomy, flexibility, strong patient relationship skills, and ability to incorporate telehealth into care.
