Brooks Blessings Adult Family Home

Responsibilities
  • Assist all residents with Activities of Daily Living (ADLs) as needed and noted in the resident's Care Plan and Brooks Blessings Daily Routine.
  • Prepare and serve meals as directed by the RM and meal plan.
  • Perform daily chores such as making beds, doing laundry, vacuuming, sweeping, mopping, cleaning kitchen after meals, doing dishes, cleaning bathrooms and toilets, and other household chores as directed by the RM or DR.
  • Dispense medications as directed by RM and the Medication Administration Record (MAR). The COD must be Nurse Delegated, pay careful attention to written instructions, and use the “Five Rights” method: right person, right medication, right dose, right time, and right route.
  • Complete a thorough shift turnover including counting and recording controlled medications upon shift start and end.
  • Engage personally with residents in the implementation of activities, monitor engagement, and proactively show care.
  • Document ADLs, behaviors, meals, toileting, activities, and other health monitoring data in Synkwise or other electronic charting systems as directed by RM. Communicate with RM or DOC about changes in condition, incidents, or other critical information that should be passed to the POA, family, or medical professionals.
Lifting Requirements

Ability to perform the following tasks if necessary:

  • Participate in physical activity.
  • Work for extended periods while standing and being involved in physical activity.
  • Perform heavy lifting.
  • Do extensive bending, lifting, and standing regularly.
Qualifications
  • Must meet state certification for Certified Nursing Assistant or Home Care Aide training program and competency with a current and in good standing credential; or have applied for Nursing Assistant Register, completed Safety and Orientation training prior to placement, and completed state certification training and passed the state exam within 120 days, having received the license within 200 days as required by state regulations.
  • Complete 12 hours of continuing education yearly to maintain caregiver license and improve skills.
  • Have a sympathetic attitude toward the care of all residents.
  • Ability to carry out directions, read and write.
  • Maturity and ability to deal effectively with the demands of the job.
