Care Manager MSW - Case Management

Description

Under supervision of Care Management Manager, works as a catalyst to promote the following: coordination of care, continuity of care, achievement of consistent and/or expected psychosocial outcomes, patient advocacy and efficient resource management. Coordinates the care for complex designated patients from pre-admission to immediate post-discharge. Acts in a non-caregiver capacity by facilitating coordination and communication between all member of the health care team, patient and family in the decision-making process to minimize fragmentation of the health care delivery system.

The Master of Social Work functions as part of a professional care coordination team of nurses and social workers and other clinicians to provide skilled psychosocial services to patients and their families. The MSW is a resource to clinical staff to assist them in evaluating patient needs for psychosocial and emotional support and to patients and families for dealing with crises, transitions and continuity of care.

• Master's degree in social work or related field
• Upon hire:
  Washington Clinical Independent Social Worker Associate License
• 3 years of social services experience with a health care related organization; experience should include a minimum of 2 years in acute care/ care coordination.

• Upon hire:
  Accredited Case Manager (ACM) Or
• Upon hire:
  Certified Case Manager (CCM)

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
