Behavioral Health Lcsw

The behavioral health LCSW will work in a locked unit. The unit primarily handles voluntary admissions with some involuntary admissions. Patients are expected to participate in a group milieu setting. High attention to safety precautions on the floor is mandatory. Many patients are admitted with suicidal ideation. Stringent contraband protocols are in place. The role extends beyond discharge planning and care coordination.

This position provides the full continuum of behavioral health services. Responsibilities include assessment, treatment planning, individual counseling, group therapy, and ongoing treatment interventions. Discharge planning is also a key component of the role. There is overlap with care management social workers around discharge planning and care coordination activities. The primary function of this role is the provision of behavioral health treatment services.

The role is aligned within the behavioral health reporting structure. Candidates should possess skills in conducting biopsychosocial assessments. This includes assisting patients with navigating the Department of Children and Families process. Coordinating services for home care and equipment following discharge is required. Documentation of pertinent information to facilitate care coordination is essential. Participation in care conference activities is expected. Referring patients to rehabilitation and long-term care facilities will be necessary.

The position involves referral to other disciplines and resources as needed. The role includes individual, marriage, and parental counseling. Knowledge of various assistance programs such as Medicaid and Medicare is beneficial. Emergency detention, guardianship, and power of attorney experience are also relevant. Understanding psychiatric disorders and treatment options for substance use is critical. Documentation reflecting care delivery, plans, and goals must be accurate.

Facilitating discharge and contingency planning is a vital part of this role. Working in a high-acuity psychiatric unit requires attention to detail and compliance with safety protocols. Collaboration with interdisciplinary support teams is essential to patient care. The position will involve working with adult patient populations experiencing various mental health challenges. Flexibility in scheduling and willingness to float between facilities may be required.
