Hospice Nurse Practitioner

The Hospice Nurse Practitioner (NP) evaluates patients for hospice eligibility through comprehensive clinical assessments, ensuring compliance with CMS, Medicare, and hospice guidelines.

They collaborate with the Medical Director, interdisciplinary team, providers, patients, and families to determine appropriate end-of-life care while documenting findings clearly.

The NP conducts face-to-face assessments for initial eligibility and recertification, reviews complex cases, and communicates care recommendations compassionately. They support staff education, stay updated on regulations, and participate in quality initiatives.

Qualifications include a Master’s or Doctoral Nursing degree, current licensure, national certification, and experience in hospice, geriatrics, or serious illness. Skills required are strong assessment, documentation, communication, and independent working abilities. The role involves community-based work, travel, and clinical assessments, with pay ranging from $54.52 to $65.66 per hour.
