Nurse Practitioner or Physician Assistant - Post Acute Care (FP, IM) Opportunity for a Family Medicine, Internal Medicine or Geriatric Medicine Nurse Practitioner or Physician Assistant to join a post-acute care facility 20 miles from Olympia, WA area.

• Monday – Friday – Set own schedule
• Weekday call requirements only – 7am – 7pm – telephone only

• Perform primary care in a post-acute setting for short term and long-term patients
• Manage chronic disease, diagnosis illness and medical conditions, and implement plan of care
• Order and interpret lab results
• Prescribe and monitor medications
• Consult with physicians and coordinate patient care with other healthcare practitioners
• Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
• Build relationships with your patients, patients’ family members, and facility staff to facilitate effective treatment delivery
• Promote healthy lifestyle changes

• Primary care experience a plus, post acute care experienced a plus, but not required
• Active and unrestricted Washington NP or PA license required
• Active and unrestricted DEA required

• Compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential
• Compensation range $93,000 - $114,000 and comprehensive benefits package including:
