Position Summary
Clinician member of the Multi Care Medical Partners multidisciplinary team delivering population-based care. The ARNP will provide comprehensive primary care services focusing on prevention, chronic disease management and health promotion for diverse patient populations. This role requires collaboration with physicians, RNs, pharmacists, social workers, behavioral health specialists and other professionals to optimize the health of Multi Care communities.

Schedule:
4 10-hour days - Minimum of two years' experience as a primary care provider preferred

- Direct Patient Care
- Conduct comprehensive assessments, diagnose conditions, develop plans, and prescribe medications for patients across the lifespan.
- Diagnose and treat acute and chronic disease using evidence‑based guidelines.
- Provide preventive care, health education, and counseling to individuals and families.
- Timely and accurate documentation, inclusive of direct patient encounters and asynchronous care.
- Participate in coverage for clinic colleagues per departmental guidance.
- Participate in after‑hours call coverage per departmental guidance.
- Multidisciplinary Collaboration
- Participate in multidisciplinary meetings, care reviews and care planning.
- Coordinate care with all members of the delivery team.
- Contribute to the development and improvement of population‑based care.
- Population Health Management
- Utilize data and risk stratification tools to prioritize patients at greatest risk and outreach.
- Engage in quality improvement focusing on health outcomes, cost effectiveness and cost reduction.
- Support transitions of care and reduce readmissions and ED use.
- Citizenship
- Demonstrate the Multi Care values of compassion, stewardship, kindness, joy, collaboration and respect.
- Adhere to Multi Care policies and processes.
- Maintain active Washington State Nurse Practitioner or Physician Assistant license.
- DEA and state prescriptive authority.
- Current BLS for Healthcare Providers certification by the American Heart Association.
