PRIOR INFUSION EXPERIENCE REQUIRED - PRIOR CHEMOTHERAPY A SIGNIFICANT PLUS!

Job Summary

Provide nursing care to a defined group of patients, adhering to Kaiser Foundation Health Plan of Washington ambulatory Nursing Care Standards, the Nursing Code of Ethics and Kaiser Foundation Health Plan of Washington policies. Coordinate and facilitate Health Care services for the patient in other segments of the Health Care delivery system. Provide and coordinate nursing care for the patient in collaboration with the Health Care team.

The Registered Nurse is an integral member of the Health Care team, overseeing and participating in patient assessment and the formulation and implementation of the care plan.

Essential Responsibilities
  • Using the nursing process, provide ongoing assessment of the nursing care needs of a defined group of patients. Use nursing diagnosis to describe this assessment. Develop a written plan of care. Assist patients and families/significant others in developing health care goals. Determine priorities of patient care. Provide nursing interventions safely and competently. Provide information preceding interventions. Provide telephone consultation. Evaluate patients response to care.

    Anticipate changes in patients status and modify the plan in collaboration with the health care team.
  • Demonstrate understanding of the physiologic and psychological effects of normal aging by assessing, planning, implementing and evaluating care for the age populations served. Demonstrate effective communication with those age populations. Identify learning needs and participate in educational programs. Collaborate and coordinate with other health care team members. Assess the needs of designated patients requiring hospitalization; initiate nursing admission database and discharge planning with inpatient staff and physician.
  • Communicate recognition of the patients and family psychosocial, cultural and emotional needs. Orient patients and family to the role of the professional nurse and other health care team members, the nursing services available, and the means of access to those services. Reinforce orientation to the Consumer Bill of Rights and Responsibilities. Identify the patients educational needs and develop an educational component for each nursing care plan.
  • Assess patient lifestyle risk factors. Integrate health promotion and preventive aspects of care with ongoing assessment. Work with patient to develop a plan to improve general health status. Implement physical and psychological preventive measures. Promote patient self-care responsibility.
  • Identify own learning needs and participate in continued education. Participate in staff orientation and student education activities. Collaborate with managers on performance reviews. Assure quality of care through development, implementation and evaluation of professional nursing standards and participation in multidisciplinary quality assurance activities. Participate in nursing research and incorporate findings into practice. Adhere to KFHPW personnel policies and maintain high professional standards.
