Position Summary

The LPN/LVN provides holistic nursing care to patients and families under the supervision of a Registered Nurse, following the Plan of Care. This role supports patients and families by anticipating changes in condition, promoting patient and family choice, ensuring continuity of care, and enhancing quality of life for patients at the end of life.

THIS IS A COLLECTIVE BARGAINING POSITION

* DAY SHIFT

* Mon-Friday 8 to 5pm

* FULL TIME

Pay Range: $25.88 - $40.29

Position Specific Responsibilities

* • Provide LPN/LVN-level hospice nursing services as established by the supervising RN.

* Observe and report patient data to the RN for use in developing the Plan of Care.

* Implement nursing interventions in accordance with the Plan of Care and hospice policies.

* Anticipate disease progression and implications for symptom management.

* Educate patients and families about care needs and anticipated changes in condition.

* Assist patients and families with understanding insurance benefits and managing care in a cost-effective manner.

* Document observations, interventions, and communications accurately and timely according to established standards.

* Manage assigned caseload and communicate Plan of Care updates to the RN as appropriate.

* Collaborate with the hospice team through effective communication and time management; apply critical thinking and problem-solving skills to address team issues.

* Maintain awareness of professional scope of practice and compliance with hospice Conditions of Participation.

* Participate in quality improvement activities and implement recommended changes as appropriate.

* Perform triage and medication desk duties as assigned.

* Adhere to Compassus/Providence policies, procedures, and applicable regulations.

Education and/or Experience

State Specific Requirements

California

* Education:

Must meet certification, licensure, or registration requirements.

* Work Experience:

Minimum 1 year as an LVN (6 months home care preferred).

* Certifications/Licenses:
California Licensed Vocational Nurse required upon hire;
Infusion and Venipuncture Certification required;
National Provider BLS - American Heart Association within 30 days of hire. Valid driver's license and auto insurance required upon request.

Oregon

* Education:

Graduation from an accredited LPN/LVN program or Physical Therapy Assistant program.

* Work Experience:

Minimum 2 years in a skilled nursing facility or medical clinic (acute care, home health, or public health experience preferred).

* Certifications/Licenses:
Oregon Licensed Practical Nurse required upon hire;
National Provider BLS - American Heart Association within 30 days of hire. Valid driver's license and safe driving record required. Criminal background check required.

Texas

* Education:

Must meet certification, licensure, or registration requirements.

* Work Experience:

Nursing experience preferred, including 1 year in hospice or working with terminally ill patients.

* Certifications/Licenses:
Texas Licensed Vocational Nurse (permanent, temporary, or compact) or New Mexico Licensed Practical Nurse required upon hire;
National Provider BLS - American Heart Association required upon hire.

Washington

* Education:

Certificate of Nursing or equivalent education/experience.

* Work Experience:

Minimum 1 year in an acute care setting (home health preferred).

* Certifications/Licenses:
Washington Licensed Practical Nurse required upon hire;
National Provider BLS - American Heart Association within 30 days of hire. Valid driver's license and auto insurance required upon request.
