In-Home Health - Nurse Practitioner or Physician Assistant; -Washington; Traveler

Job Summary

Signify Health, part of CVS Health®, is seeking a full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations.

• Build trusted relationships and bring your heart into every visit to improve patient outcomes.
• Conduct comprehensive evaluations, including medication and medical history review, physical exam, and diagnostic tests (e.g., diabetic eye exams, spirometry) when ordered by the health plan.
• Use an iPad and intuitive clinical workflow for each evaluation; prescribing medicine, ordering lab tests, or altering treatment regimens is not required.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Provide diagnostic and preventive services such as spirometry, DEE, bone density screening, and other indicated products.

Primary workplace is the private residence of members. The environment may include household pets, secondhand smoke, and potential allergens or irritants. Clinicians must perform duties safely and comfortably in these settings.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification is required.
• Multi‑state licenses are preferred.
• Eligible to serve all counties within the state of Washington.

Base pay for each in‑home health evaluation: $95,738.00 – $. Pay may vary based on experience, education, geography, and other factors.

Weekly hours: 40. Time type: full‑time.
