Signify Health, part of CVS Health®, is seeking a Full‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. You’ll engage with people in the comfort of their own homes and help bridge gaps in care. Your work will build trusted relationships to make people healthier and bring flexible, comprehensive, and personalized health evaluations right to people’s front doors.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests.

You’ll use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medicine, order lab tests or alter people’s current treatment regimen.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).

As an in‑home health clinician, your primary workplace is the private residence of our members. You must be able to comfortably and safely perform your duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi‑state licenses.
• Ability to cover primary locations: all counties within the state of Washington.

The pay range represents the base pay for each In‑Home Health Evaluation completed by a physician:

$95,738.00 – $

Pay is dependent on experience, education, geography and other relevant criteria. Additional reimbursement opportunities exist for completed Diagnostic and Preventative Services (DPS) as ordered.

Anticipated weekly hours: 40. Time type:
