Hospice RN After Hours
Puget Sound Home Health & Hospice is expanding, and we are seeking an experienced Hospice RN After Hours RN to provide high-quality after-hours hospice nursing care across Thurston, Grays Harbor, and Mason counties.
Position:
Full-Time After-Hours 7 On/7 Off Hospice Registered Nurse (RN) Case Manager
Schedule:
Monday 5:00 PM – Friday 8:00 AM and Friday 5 PM to Monday 8 AM Territory:
Thurston, Grays Harbor, and Mason County, Washington Compensation: $90,000–$110,000 annually, DOE

Key Responsibilities
- Deliver after-hours hospice nursing visits, urgent care interventions, and skilled assessments
- Complete in-person visits as required and assigned by Triage RN
- Provide pain and symptom management in accordance with hospice standards of care
- Educate patients and families on hospice philosophy, medications, and care expectations
- Document all assessments and interventions accurately and efficiently
- Communicate promptly with the interdisciplinary hospice team (IDT) to ensure continuity of care
- Participate in QAPI and quality improvement initiatives
Qualifications
- Current Washington State Registered Nurse (RN) license
- Hospice agency experience required
- Current AHA BLS certification
- Strong clinical assessment and independent decision-making skills
- Ability to work autonomously during evening, night, and early-morning hours
- Reliable, insured vehicle and valid driver's license
