About This Role

The RN Case Manager Liaison Nurse will focus on discharge planning and follow-up care for patients hospitalized at network facilities. This role is ideal for experienced nurses who excel in collaborative care and patient advocacy.

• Pay: $49 - $82/hour
• Location: Olympia, WA
• Facility: St. Peter
• Schedule: 0.6 FTE, Variable Mon-Fri, 8AM-4:30PM, Every other weekend rotation, Rotating holidays
• Sign-On Bonus: $5,000 applies to eligible external hires

• Ensure patients referred to case management meet established criteria.
• Assess patients' physical, mental, financial, and psychosocial status.
• Develop patient-centered case management plans with specific goals and outcomes.
• Evaluate the ability of designated caregivers to provide support.
• Coordinate and implement interventions using evidence-based guidelines.
• Collaborate with healthcare professionals regarding the plan of care.
• Monitor and evaluate patient responses to therapeutic interventions.
• Facilitate referrals for home health care, long-term care, and hospice.
• Collect data to evaluate the effects of care coordination on quality outcomes.
• Assist patients in understanding and complying with their medical treatment plans.

• Education:
  • Associate's Degree (ADN) required
• Experience:
  • Experienced level required
