Responsibilities
Under supervision of Care Management Manager, works as a catalyst to promote coordination of care, continuity of care, achievement of consistent and/or expected psychosocial outcomes, patient advocacy and efficient resource management. Coordinates the care for complex designated patients from pre‑admission to immediate post‑discharge.
Required Qualifications- Master's Degree in Social Work or related field
- Upon hire:
Washington Clinical Independent Social Worker Associate License - 3 years of social services experience with a health care related organization; experience should include a minimum of 2 years in acute care/care coordination.
- Upon hire:
Accredited Case Manager (ACM) or - Upon hire:
Certified Case Manager (CCM)
