Care Manager MSW

Description

Under supervision of Care Management Manager, works as a catalyst to promote the following: coordination of care, continuity of care, achievement of consistent and/or expected psychosocial outcomes, patient advocacy and efficient resource management. Coordinates the care for complex designated patients from pre‑admission to immediate post‑discharge.

• Master's Degree in Social Work or related field
• Upon hire:
  Washington Clinical Independent Social Worker Associate License
• 3 years of Social services experience with a health care related organization; experience should include a minimum of 2 years in acute care / care coordination.

• Upon hire:
  Accredited Case Manager (ACM)
• Upon hire:
  Certified Case Manager (CCM)

Pay Range: $36.00 - $56.84

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
