About the Role

Do you have a heart for seniors and a passion for supporting their emotional and social well-being? We are seeking a Social Services Assistant to join our compassionate team and help ensure that each resident receives the support they need to thrive in our care community. As a Social Services Assistant, you ll play a key role in coordinating and providing psychosocial support to residents and their families, assisting with admissions, discharges, and transitions, and advocating for the dignity and well-being of those we serve.

Key Responsibilities
  • Assist with admissions and discharges by coordinating assessments, documentation, and family communications.
  • Provide emotional support and crisis intervention for residents and families.
  • Facilitate care planning and interdisciplinary meetings.
  • Advocate for residents’ rights and personal preferences.
  • Help identify and address resident needs, including mental health, behavioral, or social issues.
  • Document assessments, progress notes, and social histories accurately.
  • Coordinate referrals to community resources and support services.
  • Participate in developing and implementing individualized care plans.
Qualifications
  • High school diploma or equivalent required;
    Associate’s or Bachelor’s degree in Social Work, Psychology, or Human Services preferred
  • Prior experience in a long-term care or healthcare setting strongly preferred
  • Knowledge of state and federal regulations for nursing homes a plus
  • Empathy, patience, and excellent communication skills
  • Ability to maintain confidentiality and professional boundaries
