* Participates in meetings and/or committees as needed to assist in decisions effecting area/unit or organization.
* Coordinates patient evaluation and screening processes.
* Establishes and maintains contact with referring providers; evaluation of patients that meet procedure criteria.
* Directly involved in patient and family education and consent.
* Conducts patient tracking and triage.
* Reviews clinical progress of patients and coordinates communication to referring, valve, and primary care physicians along with the research team as required.
* Plans, organizes, and provides direct care to the valve patient.
* Upon receipt, reviews pertinent clinical records.
* Facilitates the development of a patient specific teaching plan.
* Collaborates and coordinates the medical record review with other team members to attain positive patient outcomes based in data, research, and evidence-based practices.
* Provides oversight of valve clinic schedule and processes.
* Coordinates inter-departmental processes and communication.
* Coordinates valve program team meetings and communication processes.
* Determines and coordinates education of staff members.
* Develops and maintains clinical policies and procedures.
* Participates in NMHS performance improvement process, data collection and management.
* Implements, utilizes and maintains the Structural Heart (TAVR) database/registry.
* Provides outcomes to leadership and others as needed and requested.
* Associates Degree in Nursing (ADN) from an accredited school of nursing required.
* Bachelors of Science in Nursing (BSN) from an accredited school of nursing preferred.
* Successful completion of annual mandatory education requirements.
* A minimum of 2 years cardiology nursing experience required.
* Previous ambulatory coordination experience required.
* Strong interpersonal skills necessary to effectively communicate and interact with patients, family members, hospital personnel, and co-workers.
* Ability to multi-task and prioritize work assignments.
* Ability to learn, retain, and utilize computer skills that are required for department and hospital information system.
* Ability to think critically and problem solve.
