Physical Therapist – Skilled Nursing RehabService Line: Hillcrest Rehab Facilities – servicing multiple locations within Bellevue, Papillion, and Millard
Schedule: Weekend | Minimum of 2 days per month
Hillcrest Rehab Services provides therapy support across Hillcrest Health & Living’s skilled nursing communities, including post-acute rehabilitation, short-term recovery, and long-term care. Physical Therapists play an important role in helping patients improve mobility, strength, balance, pain management, safety, and independence.
Working closely with onsite clinical teams, Physical Therapists create individualized plans of care that help patients recover after illness, injury, surgery, or hospitalization while supporting greater confidence, function, and quality of life.

You will:
- Complete physical therapy evaluations using appropriate tests and measures to establish timely plans of care.
- Provide treatment interventions that support mobility, strength, balance, endurance, pain management, gait, transfers, and functional independence.
- Progress treatment plans using evidence-based guidelines, reimbursement requirements, and patient goals.
- Educate patients and family members using effective, culturally sensitive communication that considers individual cognitive needs.
- Complete accurate documentation and charging to support therapy services.
- Collaborate with physicians, nursing, therapy team members, and other interdisciplinary partners to support each patient’s care plan.
- Plan safe and effective transitions within or outside Hillcrest service lines, including recommended services, equipment needs, and handoff communication.
- Supervise, collaborate with, and delegate appropriate tasks to Physical Therapist Assistants, technicians, and support personnel in accordance with regulations.
- Identify patients who may benefit from additional medical or therapy services and complete screenings as needed.
- Manage an efficient patient schedule while considering safety, preferences, environmental needs, and productivity expectations.
- Identify risks related to patient care, infection control, equipment, documentation, and safety, then take appropriate action.
- Participate in professional development, clinical education, advocacy, presentations, or other activities that support the therapy profession.

- Previous experience in skilled nursing, post‑acute care, hospital, home health, outpatient, or a similar setting is welcomed but not required.
Graduate of an accredited Physical Therapy program required.
Active Physical Therapist license in Nebraska or a compact state required.
