One full-time position | 40 hours/week | Monday - Friday 8:00 am - 5:00 pm Job Duties Referral Care Coordinators assist patients by scheduling specialty medical appointments and services within and outside the clinic as referred by providers. Works collaboratively with the clinical team to serve as a liaison between the patient and specialty service providers to ensure the patient receives quality and timely care.
Ensures proper documentation in relation to outreach efforts within the database while maintaining confidential electronic health records. Maintains professional oral and written communication skills to accurately complete the referral process.
- Reviews medical records and executes medical directives: pre-ops, referrals, and scheduling appointments. Uses databases and spreadsheets to expedite the referral process.
- Schedules and coordinates appointments for One World clinics as well as off-site specialty clinics. Partners with other internal departments in the process, as needed. When coordinating appointments with off-site clinics, serves as an advocate for the patient, representing One World in a positive and professional way.
- Collaborates with the medical team to ensure the patient understands the treatment plan and financial resources.
- Maintains confidential electronic health record: documents treatment plan and ensures records are received from referring facilities. As needed, faxes pertaining to notes to referring facilities.
- Weekly, follow up and document in the database the results of the patient referrals and the outcomes. Reports on the progress of referrals on a weekly basis.
- Completes additional requirements such as, but not limited to, prior authorizations, facility forms, and insurance denials as needed by facilities to ensure prompt scheduling.
- Stays abreast of available social services within the organization and community. Refers and/or prescreens patients to appropriate services based on guidelines.
