Occupational Therapist – Hillcrest Millard
Hillcrest Health & Living is looking for a compassionate and motivated Occupational Therapist to join our rehabilitation team at Hillcrest Millard in Omaha, NE.
Hillcrest Millard is a skilled nursing community offering short-term rehabilitation and long-term care in a supportive, resident-centered setting. Our therapy team works closely with nursing, providers, residents, families, and interdisciplinary partners to help individuals regain independence, rebuild confidence, and return to the activities that matter most.

As an Occupational Therapist, you will evaluate residents and develop individualized treatment plans that support independence with daily living skills, mobility, safety, cognition, and meaningful routines.
You will use evidence-based treatment approaches while collaborating with the interdisciplinary team to help residents achieve functional goals and experience the highest possible quality of life.
You will:
- Complete occupational therapy evaluations using appropriate assessments and establish individualized plans of care.
- Deliver therapy treatments that support progress toward functional goals, safety, and independence.
- Help residents improve activities of daily living, functional mobility, self-care, cognition, and home-management skills.
- Adjust treatment plans based on resident progress, clinical judgment, and established goals.
- Educate residents and family members on treatment plans, safety strategies, adaptive equipment, and recommendations for continued care.
- Complete timely and accurate documentation, charging, and required clinical records.
- Collaborate with physicians, nursing, therapy team members, and other interdisciplinary partners to coordinate resident care.
- Assist with discharge planning, equipment recommendations, and transitions between care settings.
- Supervise and delegate appropriate care to therapy assistants, rehabilitation technicians, and support staff in accordance with applicable regulations.
- Identify residents who may benefit from additional medical or therapy services.
- Maintain a strong focus on resident safety, infection control, quality outcomes, and regulatory compliance.

- Ability to stay organized, complete accurate documentation, and manage competing priorities.
- A calm, flexible, and professional approach in changing or stressful situations.
- Previous experience in skilled nursing, post-acute care, hospital, home health, outpatient, or a similar setting is welcomed but not required.
- Graduate of an accredited Occupational Therapy program required.
- Active Occupational Therapist license in Nebraska or a compact state required.
- Basic Life Support certification through the American Heart Association required.
