Job Description

Overview

Manages cases to ensure patient rights are upheld and patients are involved in their treatment plans. Initiate and implement clinically appropriate discharge plans. Advocate for patients in accessing community, social, medical and financial resources. Work ethically and legally following organizational policies, processes, protocols and procedures. Responsible for quality service delivery and internal/external customer relations, upholding the mission and values of the department and facility.

Responsibilities
  • Compile and analyze facility, program, departmental and community data to ensure organized, quality, individualized patient care and continuity of aftercare services and clinically appropriate discharge plans, ensuring all documentation is complete and meets regulatory requirements
  • Coordinate and analyze patient medical and psychosocial data to develop social histories and facilitate a plan of care and discharge plan that meets individual patient/family needs and licensure/certification standards
  • Mentor and instruct patients and patient representatives regarding facility services and patient rights
  • Engage patients and patient representatives in treatment planning and aftercare services
  • Obtain information on previous functioning to inform the plan of care
  • Under the direction of the Case Manager, coordinate with patient representatives regarding rehabilitation team recommendations to support timely discharge
  • Instruct clinical team members in psycho-social patient information to enhance care plan development and revisions
  • Educate clinical team regarding availability of services, patient preferences and patient rights to develop a plan of care and a discharge plan
  • Negotiate with community agencies regarding patient needs and referrals to ensure continuity of care
  • Communicate with patients, patient representatives and potential funding sources to facilitate eligibility for benefits (e.g., Medicaid applications, disability benefits)
  • Mentor and educate patients and representatives regarding eligibility for and application to specific programs
  • Educate patients and representatives regarding advanced directives and legal protection to support self-determination
  • Address concerns and grievances with patients and representatives and facilitate resolution to ensure customer satisfaction
  • Collaborate with Case Manager to coordinate assignments and ensure a team approach to care plans and discharge plans
  • Assist with continuity of care in the Case Manager’s absence and take directions from Case Management Supervisor and Director as needed
  • Follow instructions from Case Management Leadership to perform other duties as assigned to achieve department goals
  • Operate telephone, dictation equipment, photocopying machine, fax, and computer to communicate and compile patient information
  • Maintain a safe environment for the department; ensure equipment and environment cleanliness and safety
Qualifications

(Education/training and/or Experience)

Bachelor's Degree in Social Work from an accredited school. One year of social work experience preferred. Experience with the elderly and/or disabled preferred. Must possess current CSW (Certified Social Worker) or obtain certification within the first three months of employment.
