Community Health Worker Program Coordinator
The position incumbent works under the direction of the assigned Division Chief or designee, coordinating, directing, and managing the operations and services of the Community Health Worker (CHW) program. Essential functions include:
- Coordinating and monitoring program activities, ensuring effective prioritization and assignment completion meeting health department goals and grant requirements.
- Coordinating, recording, and maintaining training activities for current and new CHWs.
- Establishing and maintaining effective work relationships with clients, supervisors, County employees, elected officials, attorneys, law enforcement, judges, other agencies, and the public.
- Complying with Civil Service Rules, collective bargaining agreements, County policies, department policies and laws to create a cooperative, safe, respectful and quality work environment.
- Facilitating training and peer mentoring for CHW staff, focusing on core competencies, professional boundaries, and career development.
- Coordinating staff development needs and recommending appropriate professional development opportunities for CHWs to the Supervisor.
- Coordinating logistics for community health education activities and outreach events.
- Collaborating with the Supervisor implementing Social Determinants of Health (SDOH) screening tools, ensuring clients connections to essential non-medical resources (e.g., housing, food, transportation).
- Drafting and recommending strategies supporting clients in navigating healthcare systems and advocating for health needs.
- Overseeing CHW documentation, analyzing outreach metrics identifying service gaps and reporting on program impact.
- Preparing and maintaining various records, reports, correspondence and documents.
- Maintaining job knowledge and skills (e.g. seminars, webinars, conferences, research, meetings, continuing education).
- Responding to questions and inquiries within expertise.
- Drafting and recommending standardized procedures and program policies streamlining CHW workflows and enhancing consistency of community outreach services.
- Demonstrating empathy, respect and understanding toward individuals/families and community values.
- Ensuring program readiness assigning outreach duties, monitoring field activities, and conducting meetings for program updates.
- Maintaining confidentiality of medical and personal health information.
- Providing guidance and technical assistance to subcontractors and staff.
- Promoting positive work environment and assisting in resolving conflicts.
- Participating in internal and community committees.
- Participating in public health emergency response planning and responding during emergencies.
- Safely operating a motor vehicle when required to travel on County business.
- Reporting to work with regular, consistent attendance.
- Performing other duties as assigned and directed.
Education and work experience and other requirements include:
- Bachelor's degree in Public Health, Public Health Administration, Health Care Administration, Social Work, or Management required.
- Two (2) years as a Community Health Worker or experience in community health outreach position preferred.
- One (1) year of supervisory experience preferred.
- Two (2) years of MSOffice experience required.
- Valid driver's license and own transportation upon hire and maintained throughout employment required.
- Completion of a pre-employment criminal record check and conditional offer drug screen required.

Work involves potential exposure to weather conditions. Work may be stressful when dealing with time constraints, multiple/changing priorities, limited resources and uncooperative/irate individuals.
- Work requires some physical activity, including extended periods of sitting, standing, frequent walking, reaching and occasional balancing, kneeling and bending. Work also requires the ability to frequently lift/carry objects weighing up to 10 pounds and occasionally up to 25 pounds.
- Required sensory abilities include vision, hearing and touch. Visual abilities, correctable to normal ranges, include close, distance and color vision, depth perception, and the ability to adjust focus.
