The Physical Therapist will be responsible for assessing, planning, and implementing individualized therapy programs for residents in a skilled nursing facility. The role involves working closely with a multidisciplinary team to deliver high‑quality rehabilitative care that promotes functional independence and overall well‑being.

This position operates in a clinical healthcare environment.

• Patient Assessment:
  Evaluate patients’ physical conditions, mobility, and functional abilities to develop comprehensive treatment plans tailored to their needs.
• Treatment Planning:
  Create and implement personalized therapy programs that address specific goals, improve mobility, alleviate pain, and enhance overall physical function.
• Therapeutic Interventions:
  Administer therapeutic exercises, manual therapy techniques, and modalities to help patients achieve their rehabilitation goals.
• Progress Monitoring:
  Regularly assess and document patient progress, adjusting treatment plans as necessary to optimize outcomes.

  Educate patients and their families on exercise techniques, safety measures, and home care strategies to support ongoing recovery and maintenance.
• Collaboration:
  
  Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and address any issues that may impact patient progress.
• Documentation:
  Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries in compliance with facility policies and regulatory requirements.
• Compliance:
  Adhere to all safety and infection control protocols, as well as facility policies and procedures, to ensure a safe and effective therapeutic environment.

• Education:
  
  Doctoral or Master’s degree in Physical Therapy from an accredited institution.
• Licensure:
  Current state licensure as a Physical Therapist.
• Experience:
  
  Previous experience in a skilled nursing or long-term care setting preferred, but not required.
