, offered as hourly or per‑visit rates based on your preference and experience
- Help patients improve function
, maximize independence, and minimize long‑term physical limitations - Assist patients in returning to the highest level of functional and productive living possible within their capabilities
- Collaborate with other home care professionals through interdisciplinary care planning
- Deliver prescribed occupational therapy and all related skilled procedures
- Conduct initial evaluations
, OASIS Start of Cares (SOCs), create and update plans of care
, develop maintenance programs
, and perform 30‑day reassessments - Consult with physicians regarding changes in patient condition or treatment needs
- Supervise up to two Certified Occupational Therapy Assistants (COTAs) in accordance with state guidelines and company standards
- A genuine passion for serving others and helping individuals live life to the fullest
- Graduate of an AOTA‑accredited Occupational Therapy program or successful completion of the National Certification Examination
- Eligible for, or currently holding,
NBCOT certification - Current Nebraska OT license in good standing
- At least one year of experience in an acute care setting
- Home health experience (2+ years preferred)
- Reliable transportation to visit patients in their homes
