* Our Community Resource Liaison Will:

• Guide the organization to sustainable, ethical, and profitable growth as measured by
  goals for ADC, budgeting, and financial metrics.
• Work to ensure meaningful and effective engagement and relationships with
  communities, families, and patients as measured by community utilization and feedback.
• Implement marketing and promotional initiatives that are consistent with our core values
  and help to improve our branding and the public’s perception of services offered.
• Provide educational in-services to Heritage Communities residents and associates on a
  regular basis. Become a resource for customers and the community regarding all service lines.
• Foster a culture in alignment with the Heritage Core Values as measured by associate
  feedback and surveys.
• Serve as a knowledgeable resource on all OnCare Health programs and services, providing education
  and guidance to residents, families, and community teams.
• Document all resident interactions and follow-up in the Customer Relationship Management system.
• Assist the Health and Wellness Navigators in marketing and community engagement efforts to achieve
  long-term, sustainable growth.
• Communicate daily with the Navigator team regarding resident needs and follow-up.
• Maintain standards of high-quality customer service
• Participate in department meetings, team meetings, in-services, and other related activities.
• Maintain confidentiality about all patient personal, financial, and health information including following
  HIPAA guidelines.
• Demonstrate the ability to think critically, plan, organize, and problem solve.
• Take the initiative to ensure Core Values are followed and patients, families, and communities receive
  excellent care.
• Demonstrate attention to detail, flexibility, and remains calm in a fast-paced, sometimes stressful
  environment.
• Promote a positive, supportive working environment for all team members.

Our Community Resource Liaison Has:

• 2-5+ years of experience in the healthcare industry, preferably in hospice or home health.
• A bachelor’s degree in nursing, social work, business, healthcare administration,
  nutrition, Health Sciences or a related field (preferred).
• The ability to work and communicate effectively with diverse groups including colleagues, clients, and various
  business contacts at various levels ranging from front-line employees to senior executives to patients and
  families.
• Demonstrated history of positive sales results involving meaningful customer relations that lead to
  outstanding results.
• An understanding of the services provided by OnCare Health services lines to patient, families, and caregivers.
• Intimate knowledge of home health, hospice and palliative qualification.
• Excellent verbal and written communication skills with the ability to navigate challenging and ambiguous
  situations involving emotions or conflict.
• Strong technical skills with knowledge of business software programs including Word, Excel, SharePoint,
  Outlook, Teams, and PowerPoint.
• The ability to work beyond normal working hours when necessary and maintain a flexible work
  schedule working weekdays, with call hours as assigned.
• A valid driver’s license, an acceptable driving record, and have an appropriately insured and functional/safe vehicle.
