ABOUT THIS POSITIONThe Aftercare/Resource Case Manager manages a caseload of program graduates residing on campus as well as emergency services guests. Their goal is to effectively meet with and challenge each person to identify obstacles, deal with issues, and move towards and maintain independence.

- Provide case management to Men’s Ministry program graduates, including graduate employees and emergency services guests.
- Connect graduates and guests to applicable agencies and programs according to their specific circumstances and needs.
- Make sure graduates and guests get access to the resources they need.
- Set goals with graduate and guests and hold them accountable for working toward them.
- Provide emergency crisis counseling, problem-solving, and de-escalation when necessary.
- Exemplify Jesus Christ daily, sharing the gospel or praying with others as opportunities arise.
- Bachelor’s degree in human services or a related field, or equivalent experience
- Experience case managing in a social service organization.
- Must be a team player, and have an excellent communication and interpersonal skills.
- Proficient with Google G-suite and general computer operations.
- A solid, visible relationship with Christ.
- Bachelor of Social Work (BSW).
- At least two years of case management experience in a social service organization.
- Detail oriented with strong organizational skills

Open Door Mission is a Gospel Rescue Mission founded in 1954 committed to breaking the cycle of homelessness and poverty. Each day, Open Door Mission’s campus offers 917 safe, shelter beds to people experiencing homelessness, serves over 4,747 nutritious meals to feed the hungry, and provides homeless preventive resources to more than 1,000 people living in poverty to empower them to remain in their own homes.
