• Provide 1:1 holistic support to patients and families in their homes as the heart of our care model.
• Partner with an interdisciplinary team to develop individualized care plans and guide end-of-life conversations with professionalism and empathy.
• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop, implement, and update individualized care plans with the patient and family as needs evolve.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Administer medications and treatments as prescribed.
• Rotate on-call duties and ensure seamless care transitions.
• Maintain reliable transportation, valid driver’s license, and proof of auto insurance.

• Associates, Bachelors, or Masters of an accredited nursing program (RN).
• 1+ years of recent acute care experience required; hospice/home health experience preferred.

• Schedule:
  
  8:00am to 5:00pm, Full-time position with on-call expectations.
