We are seeking a dedicated Licensed Practical Nurse (LPN) to join our nursing team, where you will have the opportunity to make a meaningful difference in the lives of our residents.
Job SummaryAs an LPN at Emerald Nursing & Rehab Mercy, you will play a crucial role in delivering high-quality, compassionate care to our residents. Your primary responsibilities will include assessing patient needs, providing direct nursing care, and collaborating with the healthcare team to ensure the best outcomes for our residents.

Responsibilities- Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
- Monitor residents' vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.
- Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident's needs.
- Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.
- Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
- Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
- Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.
- Valid LPN/LVN license in Nebraska
- Graduation from an accredited LPN/LVN program
- Previous experience in a skilled nursing facility or similar healthcare setting preferred
- Current CPR/BLS certification required
- Excellent verbal and written communication skills
- Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families
- Ability to perform assessments
