Position SummaryWe are seeking a dedicated, skilled, and compassionate Registered Nurse (RN) to join our growing clinical team. In this role, you will deliver exceptional skilled nursing care, conduct comprehensive patient assessments, and coordinate individualized care plans.

prior experience in Case Management is a significant plus
, as this role involves collaborating closely with multidisciplinary teams, managing patient transitions, and ensuring seamless continuity of care.
Provide high-quality, evidence-based skilled nursing care to patients in their home environment.
Conduct initial and ongoing comprehensive health assessments to develop and update individualized plans of care.
Coordinate care dynamically with physicians, therapists, family members, and the internal clinical leadership team.
Monitor patient outcomes, manage clinical documentation accurately, and ensure strict compliance with state, federal, and accreditation standards.
Educate patients and their families on disease management, medications, and self-care techniques to promote independence.
Education: Graduate of an accredited school of nursing (ADN or BSN).
Licensure: Current, unrestricted Registered Nurse (RN) license in the state of practice.
Certification: Current CPR/BLS certification.
Experience: Minimum of 1–2 years of clinical nursing experience (Home Health, ICU, ER, or Med‑Surg preferred).
Skills: Strong clinical judgment, excellent communication, tech‑savvy documentation skills, and a passion for autonomous patient care.
Transport: Reliable transportation, a valid driver’s license, and auto insurance.
Prior experience in Case Management
, care coordination, or utilization review.Familiarity with managing complex care plans and navigating community resources.
