Work Schedule:

Monday through Friday, 8:00 a.m. – 5:00 p.m.

The Triage Nurse will provide triage telephone service to enhance compliance with the patient’s plan of care and ensure prompt identification of patients with high‑risk conditions. This will be accomplished by sorting and prioritizing patients through assessments, monitoring, recording, and reporting changes in their condition. Triage nurses will be involved in every aspect of the patient’s health. Precision and accuracy in maintaining detailed assessment and reports are important in relaying information to the doctors or to the patients.

The triage nurse will be prepared, assist, and instruct patients for examinations and treatments and inform family members on health‑care education topics.

Responsibilities
  • Perform telephone triage to determine patient needs by recording and reporting patient condition to provider and advising patient based on provider instructions.
  • Respond to and return patient calls within 90 minutes based on medical priority 95% of the time.
  • Accurately record and report patient’s condition according to department policy 98% of the time.
  • Strictly adhere to practice protocols and consult patient’s chart while speaking with the patient.
  • Return phone messages and provide test results according to protocol at least 98% of the time.
  • Provide patient and family education to ensure adequate knowledge and recommend appropriate community resources as needed.
  • Document all lab, procedures, and appointments on tracking forms; complete tracking forms and follow up on missed appointments and procedures.
  • Assist Health Coaches by working patient lists and scheduling appointments as directed.
  • Maintain work areas to ensure healthy environmental conditions by cleaning and organizing equipment and rooms.
  • Handle prescription renewals, inquiries, and refills according to department/provider protocols; complete pharmacy requests accurately.
  • Provide age‑specific care to ensure understanding and comfort for patients of all ages.
  • Conduct nursing assessments, use nursing judgment to develop a plan of care, and determine severity of patient illness and intervention.
  • Document clear and concise notes describing phone calls utilizing SBAR (situation, background, assessment, recommendation) format.
  • Communicate between provider and patient appropriately; coordinate continuation of care through referrals to specialists.
  • Prepare exam rooms and medical equipment, including setting up instruments, stocking supplies, and performing medication reconciliation.
  • Assist providers to retrieve tests and lab work as directed, dispense medications at provider direction, and perform phlebotomy with proper labeling and handling of specimens.
  • Ensure accuracy of order entry and complete documentation without error at least 98% of the time.

Education
  • Graduate of an accredited LPN program required.
Experience
  • Minimum 1 year clinical experience required.
License/Certifications
  • Current American Heart Association or American Red Cross Basic Life Support (BLS) – strongly preferred at time of hire, required within 3 months of hire.
  • Current valid Licensed Practical Nurse (LPN) License, compact multistate license, or temporary permit while awaiting licensure required.
